Meet Manish, a 36-year-old man from Kadapa who was currently living in Indiranagar, Bangalore. Manish had been facing issues with early ejaculation and performance anxiety for a long time. He also had a history of masturbating quickly and also had been an alcoholic. Even though his sexual desire remains high, his wife lacks that post her delivery. These all put a strain on their relationship. Fixing these problems became important for them to be happy together. This story will reveal how Manish’s decision to develop his path toward wellness, with comprehensive care changed his life.
Finding the Issue
Manish had been married for five years and had been enjoying life, he had started struggling with premature ejaculation for the last 4 to 5 years. He had a sexual attachment for the last 15 years, but this issue arose right after his marriage. For the last 3 months, he was not getting to have physical intimacy with his wife, after her delivery. The situation became worse when his climax times became 0-1 minute, and he also started suffering from performance anxiety. Sometimes early ejaculation timing is used to get better with condoms. However, even after he had a good erection and also a high sexual desire, his wife lacked the desire which created an impact on their sex life. And he had never discussed any of these concerns with his wife, which could have been helpful.
Unlike many Manish was also dealing with sexual health challenges and performance anxiety. Manish also had a history of quick masturbation and he was living a full life. Even though he didn’t have any history of phimosis, he reported suffering from the sensitivity on the tip of the penis. He was reportedly not suffering any kind from any kind of stress or pressure. It’s also important to mention that Manish had a history of alcoholism, which he managed by drinking occasionally. The mix of his alcohol use and sexual health issues had been frustrating for him and his partner, which led him to seek professional help.
Manish had also consulted before about his sexual health challenges but it never worked. To solve these issues, Manish tried various herbal remedies he found on Google and YouTube, but they were unsuccessful. Realizing he needed professional help, he had sought advice before, but it didn’t help much.
Seeking Professional Help
After realizing he needed professional help, Manish found out about Allo Health while browsing Facebook on a weekend afternoon. He came across some videos that grabbed his attention and made him realize the similar struggle he had been facing. After doing a few more research on Google, he decided to call and set up an appointment. He felt encouraged to visit the Allo Health Indiranagar clinic in Bengaluru where Dr. Saachi Khanna was assigned to help him.
During his first visit in September 2023, Dr. Khanna carefully looked into Manish’s situation. He found out that Manish was dealing with premature ejaculation and was also stressed about his performance. Reflecting on their conversation, Manish mentioned, “Dr. Saachi understood my issues. It made me too focused on timing, and not enjoying the moment.” Dr. Saachi kindly reassured him, saying, “Premature ejaculation is common and can be easily treated.” She also pointed out that his history of being on alcohol is one of the causes. Manish left the clinic feeling hopeful about improving his intimate life.
Manish was also referred to Miriam Koshy for counseling therapy sessions. Both the doctor and the therapist listened to Manish’s concerns and crafted a treatment plan based on his information and test results. It’s important to note that Manish had previously seen other healthcare professionals for similar sexual health issues. In this initial consultation, he was diagnosed with premature ejaculation and performance anxiety. His history of alcohol use was concerning, suggesting that his problems could be linked to both psychological and biological factors. This resulted in a personalized treatment plan that catered to Manish’s complex health requirements, marking the beginning of a new chapter in his life.
Identifying the Cause: Diagnosis and Treatment
Manish had many reasons for his sexual health problems. Dr. Saachi told him that his alcohol use and lifestyle were affecting his sexual performance. Stress, low desire from his wife, masturbating to relieve stress, and performance anxiety also played a big part. “Your mental health can affect your sexual health,” Dr. Saachi said.
Manish remembered how he first didn’t want to deal with his problems. “I didn’t know how connected my issues were until I talked to Dr. Saachi. I thought my sexual health problems were separate, not linked to my other health or mental issues,” he said.
After starting his treatment, Manish quickly began to feel better. During his next visit, therapist Miriam firmly told him, “It’s very important to stick to your treatment plan and not stop taking your medicine without talking to your doctor. Your health is a journey, not a quick fix.” This helped Manish understand how important it is to follow the treatment plan and keep in touch with his doctor.
Dealing with Challenges in Treatment
Manish started treating his premature ejaculation (PE) with a mix of medicine, counseling, diet changes, and a healthier lifestyle. He has been given Lidonair special sprays that temporarily numb penile nerve endings, delaying ejaculation by reducing sensation during sexual activity They also block nerve impulses, preventing rapid stimulation buildup and premature ejaculation. They also block nerve impulses, preventing rapid stimulation buildup and premature ejaculation. His medicines include phosphodiesterase 5 (PDE5) inhibitors that are used to treat male sexual function problems by improving the blood flow to the male reproductive organ and selective serotonin reuptake inhibitor (SSRI) which improves mood and physical symptoms of depression and also relieves symptoms of panic and obsessive disorders methods, These both worked together to help him effectively.
His doctor and therapist assured him that the sprays and pills had few side effects and weren’t addictive, easing any worries Manish might have had. He planned to keep taking his medications as advised and to follow up with his doctor. His treatment included changing his lifestyle, continuing therapy, and managing his medications carefully.
He was also advised to:
- Discuss the problem openly with his partner. This can reduce stress and improve mutual understanding.
- Perform pelvic floor exercises regularly. These exercises strengthen the muscles that control ejaculation, which could help delay orgasm.
- Try to relax and enjoy the experience instead of worrying about performance. Being in the moment can reduce performance anxiety.
- Spend time on foreplay—aim for 5-10 minutes. This helps both partners feel more connected and prepared for intercourse.
- Consider using thick, long-lasting condoms. These can reduce sensation and help delay ejaculation.
- Understand that early ejaculation is a common issue. Recognizing it as a normal problem can alleviate feelings of embarrassment or weakness.
- Eat a healthy and balanced diet. Good nutrition supports overall physical health, which can improve sexual function.
- Exercise regularly, like walking or running, for at least 30 minutes daily. Regular physical activity can enhance stamina and reduce stress.
- Follow the treatment plan as prescribed by a healthcare provider. Adhering to professional advice is key to managing premature ejaculation effectively.
- Keep up with follow-up appointments for premature ejaculation. Regular check-ins with a healthcare provider can track progress and adjust treatments if needed.
Manish’s treatment plan also included specific exercises like Kegels exercises and the start-stop technique, along with medications and lifestyle changes. Although he faced challenges in seeing quick improvements, he remained hopeful about improving his long-term sexual health.
Manish’s experience shows that issues like premature ejaculation and performance anxiety can be well-managed with the right mix of treatments. It’s important to seek professional help, as Manish did, to greatly increase the chances of improvement. However, results can vary from person to person.
Checking Progress and Making Changes
Manish’s treatment at Allo Health included regular check-ups to monitor his progress and adjust his treatment as needed. His first follow-up was in September, followed by two more in December 2023. During these visits, Miriam checked how well Manish’s medications, counseling therapies, and lifestyle changes were working.
Allo Health reminds patients to book their next visit and helps fix any problems that might stop them from getting the treatment they need. Manish chose to buy his medications from Allo Health because they were cheaper and more convenient than other options. He also liked that the medications were delivered discreetly to his home. To ensure he remembered to take his medication, Manish set up reminders. These tools help make sure he takes his medicine regularly, which is important for long-term treatments or those that must be taken at specific times.
During one of the check-ups, Dr. Saachi noted, “We’re seeing slow progress, so we need to keep monitoring and adjusting your treatment for the best results.” Based on Manish’s feedback and how he felt, Dr. Saachi changed his medication doses and added new elements to his counseling sessions.
With each visit, Manish gradually improved, and Dr. Saachi updated his prescriptions and adjusted his medicine doses according to the changes in his health. Dr. Saachi explained, “How your body responds can change, so these regular visits are crucial. They helped us stay updated and made the necessary adjustments to your treatment.”
Miriam set counseling goals to match Manish’s improvements. These goals included:
- Checking on counseling goals – This means regularly looking at what you want to achieve in counseling and seeing how close you are to reaching these goals.
- Reviewing homework and tracking progress – This involves looking at the tasks given in previous sessions to see if they’ve been completed and noting any positive changes that have occurred as a result.
- Helping to last longer during intimacy – This point focuses on techniques and methods that can help increase the duration of ejaculation, making intimate moments last longer.
- Reducing stress and anxiety – This includes strategies to calm the mind and reduce feelings of anxiety and stress, which can negatively affect emotional well-being and sexual performance.
- Enhancing communication and intimacy with the partner – This means working on ways to better communicate with one’s partner about needs and desires, thereby improving emotional and physical closeness.
Manish was advised to consistently follow the treatment plan prescribed. With continuous medication and therapy, Manish’s sexual health significantly improved, particularly in maintaining his ejaculation. These check-ups were crucial in tracking Manish’s health improvements and highlighted the importance of sticking with the treatment and continuously seeking medical advice. This flexible approach led to significant improvements in Manish’s sexual health and overall well-being.
